hot water running cold/warm/hot?

hot water running cold/warm/hot?

The hot water to my taps/shower is running lukewarm and then very cold...it starts off very hot (very hot being normal temp) and then turns luke warm in a few seconds.
The hot water pressure is ok.
The flow of hot water is ok.
The CH is working ok.
Would be grateful for some advice in order to resolve the problem. Thank you

the system is approx 12 years old.
